You must fill out and sign both forms for every employee taking the class.
On the Employer Referral Agreement form:
ü Don’t know your DOL#? Call 586-8914, give your FEIN, and they’ll tell you.
ü Request for Training Vendor: TOBE Co.
On the Course Registration/Agreement form:
ü Name of Training Vendor (School) = TOBE Co.
ü Location = Kaneohe
ü Course Title = ServSafe
ü Class Dates = The date you selected to attend
ü Total Tuition = $171.72
ü DLIR/ETF costs = $85.86
ü Employer’s costs = $85.86
FAX the completed, signed ETF forms for Oahu residents to Carol at 586-8724,
and for Maui County residents to Stephanie at 984-2090;
OR Mail the signed forms to the Local Branch Office.
